Board of Funeral Services: Background Report Instructions
Background Check for Initial License Application
Pursuant to the New Mexico Funeral Services Act [§ 61-32-6, NMSA 1978], the following applicants for initial licensure must submit to the Department of Public Safety (DPS) state criminal history background check:
- Funeral Service Interns
- Funeral Arrangers
- Funeral Embalmers
- Funeral Service Practitioners
- Direct Disposers

The Authorization for Release of Information form must be completed and mailed to the Department of Public Safety Records office at the address above. You must include the $15 background check fee as a check or money order only.
